Your care begins with appropriate lab testing and baseline health information. In-office assessments may also include a body composition scan, vital sign check, and height and weight measurements.
You will then meet with your provider to review your results, discuss your symptoms and goals, and determine an appropriate plan of care. If medication is part of your treatment plan, it can then be ordered and started.

Elevate Health does not accept insurance for office visits or in-office services. All appointments and treatments provided directly through the clinic are self-pay.

Yes. We accept eligible HSA and FSA cards for qualifying services. Please check with your plan administrator to confirm eligibility and coverage.

Yes. If you prefer to use insurance for lab testing, Elevate can provide a lab order for you to take to an outside facility that accepts your insurance.
The outside facility will bill your insurance directly. Coverage varies by plan, and Elevate cannot guarantee that all requested lab testing will be covered.

Some insurance plans provide coverage for weight loss medications. When appropriate, prescriptions can be sent to a commercial pharmacy that will submit the medication to your insurance.
Many plans do not cover weight loss medication. When coverage is unavailable, compounded medication options may be discussed based on your treatment plan and needs.

A new patient consultation with labs completed through Elevate is $160.
If you have completed the required lab work within the past three to four months, your new patient consultation is $110.
For weight loss patients paying out of pocket, there is no separate follow-up appointment fee. Follow-up visits are included in the cost of the medication.
For weight loss patients using insurance coverage, follow-up appointments are $60 due to the additional coordination required for prior authorizations and commercial pharmacy processing.
Medication costs vary depending on the medication, dosage, pharmacy, and insurance coverage. The Elevate team will review available options and associated costs before treatment begins.

Yes. Lab testing is required before medication can be prescribed for weight loss, HRT, or TRT. Follow-up labs may also be required to monitor your progress and guide treatment adjustments.

Yes, as long as the labs were completed within the past three to four months. Please provide a copy of your results for review. Your provider will confirm whether they include all necessary testing or if additional labs are required.

Yes. Elevate Health provides primarily in-office care in Moulton, Alabama, with telehealth appointments available for eligible weight loss and hormone replacement therapy patients throughout Alabama.
The Elevate team can help determine whether your care can be completed through telehealth or requires an office visit.

Procedures such as IV therapy, injectable treatments, body composition scans, and hormone pellet insertion require an in-office visit.
Some consultations, medication follow-ups, and lab reviews for weight loss and HRT may be completed through telehealth when appropriate.

Weight loss follow-up appointments are generally scheduled every one to two months.
HRT follow-up appointments are generally scheduled every three months, with lab testing completed approximately twice per year and as needed.
The timing of follow-ups may vary based on your treatment, lab results, symptoms, and progress.

Elevate offers bioidentical hormone replacement therapy options, which may include estradiol patches, micronized progesterone, testosterone cream, and hormone pellets. Elevate does not prescribe synthetic hormones.
Your provider will review your symptoms, health history, lab results, and goals before recommending the most appropriate treatment option.

Hormone pellet therapy is one form of bioidentical hormone replacement therapy. Bioidentical HRT may also include patches, creams, micronized progesterone, or other options based on your individual needs.

Hormone pellet insertion is completed in the office using a local anesthetic. The pellet is placed beneath the skin, typically near the hip.
The procedure generally takes less than 10 minutes. Your provider will explain the process, answer your questions, and provide instructions for aftercare.

Hormone pellets typically release medication over approximately three to six months. When it is time for new pellets, another brief in-office appointment will be required.
The recommended timing will depend on your symptoms, lab results, and response to treatment.

Testosterone replacement therapy options may include testosterone injections, topical testosterone creams, and pellet therapy.

When clinically appropriate, Elevate provides individualized thyroid treatment and ongoing monitoring based on your needs.

Available treatments vary, and not every infusion or injectable is appropriate for every patient.

Yes. For eligible patients, Elevate Health can order NAD+ for at-home use through a 503A compounding pharmacy.
This may provide a more convenient option for patients who are unable to visit the office for regular injections. Your provider will determine whether at-home treatment is appropriate and explain how to use it safely.

Treatment options may include oral or sublingual medications such as sildenafil or tadalafil, vacuum therapy, and injection therapy such as Trimix.
Treatment recommendations will be based on your health history, concerns, and individual needs.
